In this one-shot, the authors present their theory of the assassination of President John F. Kennedy. Was the crime planned and executed by a web of conspirators that included the Cubans, organized crime, the CIA, and others in positions of authority?

The comic includes chapters on the assassination itself, Lee Harvey Oswald, and Jack Ruby. It also purports to list inconsistent facts ignored or buried by the investigating Warren Commission. Was there only a single bullet? Were autopsy photos forged? Why are certain key cells in the famous Zapruder film missing?

Who Really Killed JFK? also contains a short bibliography of sources, and a list of more than 30 people connected with the assassination, who have since died, often under mysterious circumstances.
